Medical Card Certification Process

Get an Illinois Medical Card application completed in just two telemedicine visits!

1. REQUEST APPOINTMENT

Fill out the form above and one of our schedulers will reach out to you ASAP. Look for an email within 24 hours and a phone call within 1-2 business days.

2. SCHEDULE APPOINTMENT

An intake specialist will contact you to schedule your appointments. A minimum of two TELEMEDICINE visits are required to establish a patient-physician relationship. We take insurance!!

3. PHYSICIAN CERTIFICATION FORM

We will provide a signed Physician Certification once you complete your appointments. You will use this form to apply for your Medical Card.

4. SUBMIT APPLICATION

Once you receive your Physician Certification, you will need to complete the online IDPH Patient Application. We will provide the application link and instructions.

  1. You must be able to provide proof of your Illinois residency
  2. You cannot have a Commercial Driver’s License (CDL)
  3. You must have a diagnosis that qualifies in Illinois
  4. You must submit a signed Physician Certification with your IDPH application – and that is where we come in! We will provide the Physician Certification and then you can apply to get your card!

    1. If you feel you have symptoms of PTSD but have never been officially diagnosed, click here for a PTSD evaluation done by telemedicine.

    1. Physician Appointment Fee – due at the time of the visit
      1. We take insurance – Click HERE to see if you can use your insurance. 
      2. If you are unable to use insurance, the self-pay fee is $175 per office visit. 
      3. If you are a veteran or on medicaid, the fee is reduced to $100 per office visit with proof of veteran or medicaid status.

 

    1. IDPH Application Fee – due when you complete the IDPH online application 
      1. $100 per year, $250 for a three year card 
      2. 50% off for veterans and people on disability 

Applying for your Medical Card in Illinois is now easier and faster than ever! You can complete your telemedicine visits with us within one week. Once you complete the required telemedicine visits with our doctors, we will provide you with the Physician Certification. Then you will be able to submit your IDPH application online and the state will provide a temporary card that you can start using within 24 hours.
